Notice of Infringement – Claim
- A signature of the copyright owner or authorized agent;
- Identification of the copyrighted work being infringed;
- Details to help locate the infringing material;
- Contact information of the complaining party;
- A statement asserting unauthorized use of the material;
- An affirmation of the accuracy of the information provided.

Counter Notification – Restoration of Material
If material is taken down due to a copyright claim, a counter notification can be given to restore the material. This notification must include:

- Description of the removed material and its original location;
- Statement affirming belief that removal was by mistake;
- Your contact information and consent to jurisdiction.
